Does MDMA cure PTSD? What the phase 3 trial really found

- A LEAD, NOT APPROVED
MDMA is often marketed as a breakthrough PTSD treatment. A randomized, double-blind, placebo-controlled phase 3 trial published in Nature Medicine (2023) tested MDMA-assisted therapy against placebo with identical talk therapy in 104 adults with moderate-to-severe PTSD across multiple sites. PTSD symptom severity (CAPS-5) dropped about 24 points with MDMA-therapy versus about 15 points with placebo — a statistically significant difference (P under 0.001). Functional disability improved more in the MDMA group as well, and there were no deaths. However, severe treatment-emergent adverse events occurred more often with MDMA (about 9% versus 4% with placebo).
The catch: the study was funded by MAPS, an advocacy organization; MDMA's noticeable effects make true blinding difficult; the therapy component itself may drive part of the benefit; and the FDA has not approved MDMA-assisted therapy for PTSD. MDMA is a promising lead, not an approved cure — talk to a doctor about PTSD treatment options.
